Please fill out paper listed under the "New Patient Paperwork" AND the "PreferredOne Paperwork". 
We apologized for the amount of forms needed.  These forms are requested and required by your PreferredOne insurance.  We believe this is more paperwork than necessary, however your insurance feels it is necessary.  Please fill out all forms that pertain to your symptoms.  If you have pain that extends down your arm, please fill out the arm form.  If you do not have headaches, you do not need to fill out the headache form.  If you have any questions regarding which forms to fill out, please us at 507-645-8846.
